  1. This revival originally produced by the Old Globe Theatre, San Diego, California.
  2. When this revival opened at the Marquis Theatre on March 3, 1994, it starred Victor Garber and Bebe Neuwirth as Applegate and Lola. Jerry Lewis and Charlotte d'Amboise stepped into the roles on March 12, 1995, and stayed with the show until it closed on August 6, 1995. A videorecording of the earlier cast, taped on June 8, 1994, is available on NCOV 1677.
