  1. This revival was originally produced by the Old Globe Theatre, San Diego, California.
  2. When this production opened on March 3, 1994, it starred Bebe Neuwirth and Victor Garber in the roles of Lola and Applegate. Charlotte d'Amboise and Jerry Lewis stepped into the roles on March 12, 1995. A videorecording of the later cast, taped on Aug. 5, 1995, is available on NCOV 1821.
